I am currently accepting Anthem Blue Cross/Blue Shield insurance plans. Check your plan co-payment before your first visit.
If you are covered by another insurance plan, I am an out-of-network provider. That means you will have to file claims for reimbursement yourself.
Do You Have a Sliding Scale?
If you are uninsured, I am currently offering slding scale fees based on your ability to pay. Please contact me for more details.
How can you pay for the help that you need?
I can file Anthem insurance claims for you. You can submit a claim to any other insurance company yourself for reimbursement, after you pay for a session. Check your plan for details.
You can pay with cash, check, debit, or credit card. If you use your flexible spending or health savings account, you will need to pay for your session and file a claim with your FSA/HSA. Unfortunately, becasue of HSA rules, I cannot accept Health account debit cards.
Your Insurance and Health Savings Account
Services may be covered in full or in part by your health insurance, if you submit a claim, or your employee benefit plan. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per therapy session?
- Is approval required from my primary care physician?
Your appointment is being reserved for you! If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay the full cost of the session.
